Apply for a new bespoke permit

Below are the application forms for new bespoke permits and the guidance notes to complete the application forms. All applicants (except those applying for a small scale sewage discharge only) must fill in part A and F and complete the appropriate part B sections.

If you are applying for a small scale sewage discharge (less than five cubic metres per day) you only need to fill in part B6.5 and no other parts of the application pack.

The application forms below can be filled in electronically. Please note: You will need Adobe Version 7 or later to be able to use the electronic forms. Alternatively, you can print a copy of the application form and complete it by hand.

Please note: Once you have selected the right parts of the form you only need to fill in the sections of that form which are relevant to your application. However, you will need to go to each section and tick the appropriate boxes which will allow you to move on to the next section.

Part A

All applicants must fill in part A - About you.

Part F

All applicants must fill in either part F1 or F2.

If you are applying for an installation, waste operation, mining waste operation or groundwater activity, for example spreading to land, fill in Part F1.

If you are applying for a standalone point source water discharge fill in Part F2:

Applicants for new permits must also fill in the relevant sections of Part B. Please select the appropriate part B form from those listed below.

Once you have completed part B2 please now complete one or more of the following:

Part B3 installations (specific information)

Part B4 waste operations

Part B5 mining waste operation

Part B6 standalone water and groundwater discharges

Part B6.5 small scale sewage discharges (less than five cubic metres per day)
